Safety and feasibility of prehospital thrombolysis carried out by paramedics.

چکیده
The benefits of thrombolysis in patients with acute myocardial infarction are time dependent, with a potential 48% reduction in mortality if treatment is received within an hour of onset of symptoms. The benefits diminish thereafter. However the “call to needle” standard set by the national service framework has been achieved for only a third of patients nationally because of lengthy ambulance journeys and delays in hospital. Prehospital thrombolysis may be the most feasible means of achieving timely reperfusion, and all published studies have shown the accuracy of diagnosis, patient selection, and administration of thrombolysis. The Joint Royal Colleges Ambulance Liaison Committee favours autonomous prehospital thrombolysis by paramedics as a long term objective. We tested the feasibility of physician assisted and autonomousmodels of paramedic prehospital thrombolysis.

What are the beliefs and attitudes of paramedics to prehospital thrombolysis? A questionnaire study.
BACKGROUND The Ambulance Services have a critical role in the management of acute myocardial infarction (AMI). Paramedic delivered prehospital thrombolysis (PHT) has been proposed. To the authors' knowledge there has been no research carried out to ascertain the views of paramedics. METHODS The authors conducted a postal questionnaire study of 250 paramedics in the West Yorkshire Metropolitan...

OBJECTIVES To explore paramedics' attitudes to administering prehospital thrombolysis (PHT). METHOD In-depth interviews with 20 paramedics were recorded and transcribed and analysed for emergent themes using the constant comparative method. RESULTS Although there was a will to provide PHT because of its benefits to patients, its associated risks, aspects of pay and working conditions, and c...

INTRODUCTION Paramedics often provide advance notice of patients with suspected acute myocardial infarction (AMI) so that emergency department (ED) staff can prepare for early aggressive management and expeditious thrombolysis, but the validity of this practice is unclear. Our objective was to determine the accuracy of prehospital AMI diagnosis by Paramedic Level III (ALS) attendants. METHODS...

OBJECTIVE Heart disease is the major cause of death in Wales. Myocardial infarction accounts for most fatalities either acutely or as a result of late heart failure and unheralded sudden cardiac death. Prompt relief of new coronary occlusions by thrombolytic agents has been shown to reduce significantly both early mortality and subsequent morbidity from acute myocardial infarction. The prehospi...

Prehospital emergency care personnel can play a crucial role in the identification and management of patients presenting with ST Elevation Myocardial Infarction (STEMI). Specifically, prehospital thrombolysis by emergency medical personnel such as paramedics/nurses/prehospital doctors has been identified as being a successful and safe approach for the provision of accelerating reperfusion strat...
